Accountable Care Organizations (ACOs) are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients.
The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors.
On April 1, 2012, Premier ACO Physicians Network joined The Centers for Medicare & Medicaid Services (CMS)’ Medicare Shared Savings Program (Shared Savings Program) under the Affordable Care Act. We are a group of primary and specialty care physicians who have come together and made a commitment to take accountability for the quality care of the Medicare FFS population. We are currently serving about 9,000 beneficiaries and our mission is to provide them with a strong health support system to ensure patients are getting the right care at the right time in the right setting.
